All patients should bring insurance card. New patients should arrive 15 to 20 minutes early to complete paper work and bring any pertinent medical records or have those requested and sent ahead. Return patients should arrive 5 minutes prior to appointment to confirm demographic information.

Typically, maintenance medications are filled at the time of the appointment with enough refills until the next appointment. Controlled substance medication refills should be requested 1 week prior to end of the prescription. All other medications should be requested electronically by your pharmacy or requested through the patient portal.
